NEW PATIENT REFERRALS
Roper St. Francis Healthcare and Charleston Oncology have partnered to provide innovative and coordinated medical oncology services across the Lowcountry. Charleston Oncology’s twelve physicians supported by numerous advanced practitioners and skilled clinical staff members provide comprehensive oncology and hematology services at six office locations. We offer innovative, fully coordinated cancer treatments including access to clinical trials, immunotherapy, DigniCap, and one of only three Bone Marrow Transplant programs in the state.
Roper St. Francis Healthcare provides a multidisciplinary network of medical oncologists, radiation oncologists, interventional and diagnostic imaging, surgical oncology, and reconstructive surgeons sharing one electronic medical record.
STAT appointments are available, and we are committed to seeing all cancer patients within 72 hours of receiving their referral.

NEW PATIENT REFERRAL FORM
Instructions:
- Ask the referring secretary to fax the following reports if pertinent: PATHOLOGY, PROGRESS NOTES, X-RAY REPORTS (to include any CT scans), copies of primary and secondary health insurance cards, and LAB RESULTS TO 843-577-6523. Have them put to your attention. MUST HAVE DOB ON THE INSURED IF NOT THE PT.
- If this is an out-of-town referral, ask them to send the actual slides and x-ray films to our office (this includes CT scans).
